Survey templates

✓ Cloud or on-premise · ✓ Kiosk, QR, SMS, email and web · ✓ Live in one day

These survey templates give you the exact questions, the answer scale for each one, a three-question kiosk version and a full version for QR codes, SMS and email. Each template also explains where to run it, how to analyse the results and how to follow up on low scores. Copy them freely or build them in Qmeter.

How these templates are built

Most survey templates online are a list of questions. That is the easy part. What decides whether a survey produces useful results is everything around the questions: which scale to use, how short the kiosk version must be, which moment to ask at, how to segment the results and what happens when a customer gives a low score. Every template in this library therefore follows the same structure:

  1. The full version — six to twelve questions, each with its answer type (0–10 scale, 1–5 smileys, stars, multiple choice, open text) and a note on why it is there.
  2. The short version — three questions for a kiosk or tablet, where people answer on their way out.
  3. When and where to run it — the right channel for each moment: kiosk at the exit, QR code on the table, SMS after the visit, email after the stay.
  4. How to analyse it — the headline metric, a worked example and the segments that matter (location, staff, shift, visit type).
  5. Closing the loop — alerts, tickets and response targets for low scores.

The templates are written for organisations that serve people in person as well as online — branches, stores, clinics, hotels, airports and public service centres — but they work just as well for purely digital services.

General customer feedback templates

Start here if you need one survey for the whole business, or an always-open feedback form:

Templates by metric

Each metric answers a different question. Net Promoter Score measures loyalty, CSAT measures satisfaction with a single interaction, and customer effort score measures how easy it was to get something done. If you are not sure which one you need, our comparison of NPS, CSAT and CES explains the difference, and the explainers on NPS, CSAT and customer effort score go deeper.

Templates by industry

Industry templates use the same metrics but ask about the moments that matter in that setting — waiting and communication in a clinic, cleanliness and room comfort in a hotel, product availability in a store, touchpoints in an airport terminal:

How to choose the right template

  • You want one number for management: start with the NPS survey template for loyalty, or the CSAT survey template for day-to-day service quality.
  • You run locations and want to compare them: use the industry template closest to your business; all of them include a kiosk version and location-level analysis.
  • Customers come to you to complete a task (applications, payments, returns, support): add the customer effort score template.
  • You want customers to be able to reach you at any time: publish the customer feedback form template alongside your survey.
  • You want to hear from your own staff: use the employee NPS template.

Whatever you choose, keep the first question identical across every channel and location. That single shared question is what makes kiosk, QR, SMS and email results comparable. For more question ideas by goal, see the best customer feedback survey questions.

Using the templates in Qmeter

Every template here can be rebuilt in a few minutes with Qmeter's drag-and-drop Template Builder, which includes smiley, star and 0–10 rating components, choice and text questions, media and voice feedback, and show-if logic. A published template can run on kiosks (online or offline), QR codes, SMS and email campaigns and web links at the same time, in more than 100 languages, with real-time alerts and automatic follow-up tickets for low scores. You can try it during the 14-day free trial, or see how it works for feedback kiosks, NPS and CSAT.

Frequently asked questions

Are these survey templates free to use?

Yes. You can copy the questions and scales into any survey tool or onto paper. Building them in Qmeter adds kiosks, QR codes, SMS, email, alerts and ticketing.

How many questions should a survey template have?

Three for kiosks and tablets, and six to twelve for QR codes, SMS links and email. Every template in this library includes both versions.

Which survey template should I start with?

If you need one survey for the whole business, start with the customer satisfaction survey template. If you need a single loyalty number for management, start with the NPS survey template.

Can I combine templates?

Yes. Many organisations run an industry template at their locations, a relationship NPS survey by email every few months and an always-open feedback form. Keep the first question identical where you want to compare results.

Can the templates be used in other languages?

Yes. Translate the questions but keep the scales and structure the same in every language so the results remain comparable.
